Error: server failed to transmit file 'AY&SY?v?'
Host_Error: UserMsg: Not Present on Client 227
Any ideas what this errors could be?
I get the feeling someone (a player) is intentionally crashing the server

We have a single sticky thread in this forum, and it's about this same thing

Re: Error: server failed to transmit file 'AY&SY?
Thanks Soja for ending the run around game.
Open a support ticket asking for help
Get told (I'm paraphrasing here) "I don't know, try the forum"
Post the issue on the forum
Get told (again I'm paraphrasing) "check out another thread"
Post a 'whiney retort'
Get given a link to the 'potential' resolution
This is like a treasure hunt, except for treasure at the end it's a big pile of "sort that crap out yourself"
You guys are getting $70+ a month from me and my clan for a managed and unmanged VDS, that's nearly what I pay for cable TV!
I'm getting a bit fed up with the 'disable all your plugins' approach to every server issue I have...

Re: Error: server failed to transmit file 'AY&SY?v?'
You are being told that because it IS probably a plugin problem. Stock game servers have little to no issues. You should be able to take responsibility for configuring and troubleshooting your game servers. NFO provides as much of a managed service they can, but to have complete knowledge of all plugin configurations and combinations is simply not possible.
I pay well over $1k/m for my services here, and if there is a problem, I diagnose it. Usually it is a problem with my plugins or configuration, if not I ticket it and it gets resolved.
Unfortunately I am not familiar with GoldSrc engine issues, but I did see some useful solutions in the thread John mentioned, have you looked at it?

Re: Error: server failed to transmit file 'AY&SY?v?'
Thanks Soja,
You've helped me a few times on this forum and I really do appreciate that.
My beef is with the customer service as much as the service issue.
I still feel this could have been handled a lot better.
