The game seems to have problems when the server.cfg is executed too soon after a server is started, which happens frequently here (likely due to our very fast hardware). Try adding this to the beginning of your server.cfg or autoexec.cfg:

wait 1000

Future servers will include this line in autoexec.cfg.
